Carbocysteine Syrup: Effective Mucolytic for Respiratory Relief

Mechanism of Action

Carbocysteine syrup is a mucolytic agent that works by altering the composition of mucus, making it less viscous and easier to cough up. It achieves this by breaking disulfide bonds in the glycoprotein molecules of mucus, reducing its viscosity. Additionally, carbocysteine helps restore the balance between sialomucins and fucomucins, which are glycoproteins that regulate the viscoelastic properties of bronchial mucus. This action is particularly beneficial in conditions characterized by excessive or viscous mucus production, such as chronic obstructive pulmonary disease (COPD) and cystic fibrosis.

Uses

Carbocysteine syrup is used for the treatment of respiratory tract disorders characterized by excessive, viscous mucus. It is particularly effective in conditions such as:

• Chronic obstructive airways disease (COPD)
• Acute and chronic bronchitis
• Bronchial asthma
• Upper respiratory tract inflammation (e.g., pharyngitis, laryngitis)
• Cystic fibrosis
• Bronchiectasis
• Pulmonary tuberculosis
• It is also used to aid in the drainage of mucus in conditions like chronic sinusitis and otitis media in children.

Dosage and Administration

Adults (including the elderly):

• Initial dose: 15 ml three times a day (total 2250 mg/day)
• Maintenance dose: 10 ml three times a day (total 1500 mg/day) once a satisfactory response is obtained.
• Children:
• 2 - 5 years: 1.25 - 2.5 ml four times a day.
• 6 - 12 years: 5 ml three times a day.
• Carbocysteine syrup is contraindicated in children under 2 years of age.
• Method of Administration:
  Carbocysteine syrup should be taken orally, preferably between meals. It comes with a measuring device to ensure accurate dosing. To use the oral dosing syringe:

1. Check the measurements on the syringe.
2. Push the plunger down and insert the syringe into the medicine bottle.
3. Pull back the plunger to fill the syringe to the correct mark.
4. Place the syringe end into the mouth against the cheek and gently press the plunger down to release the medicine.

Storage

Carbocysteine syrup should be stored at room temperature, typically between 15°C and 30°C (59°F to 86°F). It should be protected from freezing and kept in a dry place. Store the syrup out of the reach of children.
